Each fiscal year, all clubs & districts that are part of the GCFP are required to fill out and send a copy of the 501c3 Form 990 Information to Judy Jackson. You must complete and submit this form to be included in the GCFP 501 C-3 Federal Income Tax Reporting Form (Charitable Organization Status). If you do not return your club's form, you will be responsible for filing any required reports for your club with the Internal Revenue Service.
First, it would really help if you have Adobe Reader 9.0 or 9.1, this is free software.

Then, you can email the forms directly to the Chairman from Adobe Acrobat.
Emailing Instructions:
If you see an envelope icon in the toolbar, click on it.
If you don't have the envelope icon, click your cursor on "File" at the top toolbar next to "Acrobat".
If you aren't comfortable with emailing the form, print a total of 2 copies of the form, one for your files and mail one to the Chairman. Her mailing address is on page 1 of the forms.
Please direct any 501c3 Form 990 questions to the Chairman.
The IRS does want each club to update its address when there is a change. This also applies to the Districts within GCFP.
We have two options to update our addresses
1. We can phone the IRS to report an address change. A club officer must call and confirm that she/he is authorized to make the change (they are if they are an officer of the club).

2. The second option is an IRS form.
The form is IRS Form 8822. It is simple form. COMPLETE ONLY PART II.

Note: If your club uses a PO Box as it's address, and the mail for the club goes to that mailbox, then you don't have to change the address each time the president or other responsible officer changes.
