IRS Forms & Information
Each fiscal year, all clubs & districts that are part of the GCFP are required to fill out and send a copy of the 501c3 Form 990 Information to Judy Rach. You must complete and submit this form to be included in the GCFP 501 C-3 Federal Income Tax Reporting Form (Charitable Organization Status). If you do not return your club's form, you will be responsible for filing any required reports for your club with the Internal Revenue Service.
INSTRUCTIONS:
When you open the form, you will be able to type the required information in the blue fields of the form, even the signature line. After you've filled out the pages for your club/district, you should print a copy for your records. Then, you can email the forms directly to Judy Rach from Adobe Acrobat:
If you see an envelope icon in the toolbar, click on it.
- An email with the subject line already filled out and the forms will be attached in the email page area will appear.
- Fill out the TO line with Judy's email address (it's on page 1 of the form, so copy it or write it down).
- Then hit the send button.
If you don't have the envelope icon, click your cursor on "File" at the top toolbar next to "Acrobat".
- Go down the column to "Attach to Email..." click on it.
- An email with the subject line already filled out and the forms will be attached in the email page area will appear.
- Fill out the TO line with Judy's email address (it's on page 1 of the form, so copy it or write it down).
- Then hit the send button.
If you aren't comfortable with emailing the form, print a total of 2 copies of the form, one for your files and mail one to Judy. Her mailing address is on page 1 of the forms. You could also use fax if you have it handy, her fax telephone number is on page 1 of the forms.

- Change of Address with the IRS
The IRS does want each club to update its address when there is a change. This also applies to the Districts within GCFP.
We have two options to update our addresses
1. We can phone the IRS to report an address change. A club officer must call and confirm that she/he is authorized to make the change (they are if they are an officer of the club).

2. The second option is an IRS form.
The form is IRS Form 8822. It is simple form. COMPLETE ONLY PART II.
Mail a copy of the form to:
- Department of the Treasury
- Internal Revenue Service
- Cincinnati, OH 45999-0023
Note: If your club uses a PO Box as it's address, and the mail for the club goes to that mailbox, then you don't have to change the address each time the president changes.
